LIGHTHOUSE JIU-JITSU ACADEMY
LIABILITY WAIVER & RELEASE OF LIABILITY
I am voluntarily participating in Brazilian Jiu-Jitsu training, martial arts instruction, strength and conditioning, and related private or group activities (“the Activity”). I understand these activities may be strenuous and involve the risk of serious injury. I recognize that training and exercise involve risks to the musculoskeletal and cardiorespiratory systems, including but not limited to sprains, strains, fractures, dizziness, fainting, abnormal heart rhythm, breathing discomfort, abnormal blood pressure response, and in rare instances, heart attack, stroke, paralysis, or death. I acknowledge that not all risks can be known in advance.
WAIVER AND RELEASE
For myself, my heirs, executors, administrators, assigns, and personal representatives (collectively “Releasor”), I knowingly and voluntarily waive any and all rights, claims, or causes of action arising from my participation in the Activity.
I release and forever discharge LIGHTHOUSE JIU-JITSU ACADEMY , its owners, members, managers, instructors, staff, volunteers, affiliates, agents, representatives, successors, and assigns (collectively “Releasees”) from any physical, emotional, or psychological injury, illness, damage, or loss I may suffer as a result of participation.
INDEMNIFICATION & HOLD HARMLESS
I agree to indemnify, defend, and hold harmless the Releasees from any claims, demands, damages, liabilities, costs, or attorney fees arising from my participation or brought by me or on my behalf.
MEDICAL CARE AUTHORIZATION
If I require medical care during participation, I authorize LIGHTHOUSE JIU-JITSU ACADEMY to provide or arrange emergency care deemed necessary, including first aid, CPR, AED use, emergency transport, and sharing relevant medical information with medical personnel.
I accept full financial responsibility for any medical treatment received and understand that I am responsible for maintaining my own health insurance coverage.
ASSUMPTION OF COSTS & PROPERTY DAMAGE
I agree to be financially responsible for any damage to equipment or facilities caused by my willful actions, negligence, or reckless conduct.
RESPONSIBILITY FOR THIRD PARTIES
I acknowledge that LIGHTHOUSE JIU-JITSU ACADEMY is not responsible for the acts or omissions of any independent contractor, visiting instructor, event organizer, or third party conducting activities on its behalf.
PHOTO & VIDEO RELEASE
I grant LIGHTHOUSE JIU-JITSU ACADEMY permission to photograph or record me during training or events and to use my likeness in print, digital, and web-based materials for lawful promotional or educational purposes without compensation.
I understand these images may be edited, published, or distributed without further approval. I waive any right to inspect or approve final products and waive any claim to royalties or compensation.
MINOR PARTICIPANTS
If the participant is under 18 years of age, the parent or legal guardian agrees to all terms of this waiver and consents to the minor’s participation.
GOVERNING LAW
This agreement shall be governed by and interpreted under the laws of the State of Michigan.
AGREEMENT
I certify that I have read this waiver, fully understand its contents, and agree to its terms voluntarily.
